  2. Ensure that the application is FULLY completed using CAPITAL letters for legibility. Forms not properly completed will not be processed. All sections must be completed, however, for sections not applicable please indicate “N/A”.
  3. A valid working email address and one mobile phone number are mandatory*.
  4. Applicants are responsible for ensuring that all supporting documents are submitted with this form.

  3. There should be no background shadows
  4. The length of the face in the photograph should be 25mm to 35mm. There should be a margin of 3mm to 4mm between the head and the edge of the photograph
  5. A full frontal pose of head, neck, and top of the shoulder is required
  6. There should be no creases or ink marks on the photograph
